On Tuesday 6 November the national Better Care support team is visiting the city to discover how Hull City Council and NHS Hull Clinical Commissioning Group work together to support some of the most vulnerable people in our city.
The Better Care Fund is a countrywide programme spanning both the NHS and local government which seeks to join up health and care services, so that people can manage their own health and wellbeing and live independently in their communities for as long as possible. The team will be visiting the Jean Bishop Integrated Care Centre and meeting senior leaders from the council and CCG.
Councillor Gwen Lunn, portfolio holder for adult services, said: “We look forward to welcoming the national Better Care team to Hull so that we can highlight the fantastic partnership work taking place between the CCG and the Council.
“Working closely with the CCG gives us greater opportunities to deliver a number of joint initiatives and services to address the rapidly changing needs of adult social care in Hull.
“The strengthened relationships and collaboration has enabled us to plan and redesign services to support vulnerable adults, and our joint approach provides some solutions to pressures in adult social care by using resources effectively, avoiding duplication and improving people’s experience of both health and social care.”
Erica Daley, Director of Integrated Commissioning at Hull Clinical Commissioning group said:
“We are really pleased that the national team has taken an interest in the joint work we have been doing to take our Better Care Fund plans forward in Hull over the last five years. The Jean Bishop Integrated Care Centre is central to our Better Care Fund plans, supporting the health and social care partnership to prevent ill health and keep older people well for longer.
“Geriatricians, and other clinicians, working alongside local authority colleagues at the Centre are able to get the full health picture and discuss people’s underlying social issues and concerns - something there isn’t always time for in a busy clinic environment. It has been fabulous hearing how different it feels for the patients coming in for assessment.”
